Credit Roles for Surgery Residents

Four roles are possible (official ACGME interpretations in parenthesis):

SC = Surgeon Chief Year (only cases credited as surgeon during 12 months of Chief Year)
SJ = Surgeon Junior Years (all cases credited as surgeon prior to Chief Year)
TA = Teaching Assistant (more senior resident working with junior resident who takes credit as surgeon)
FA = First Assistant (any instance in which a resident assists at an operation with another surgeon---an attending or more senior resident-responsible for the operation)

Some specific notes:

  1. SC should only be used during yr 5
  2. SJ should be used for most cases in yrs 1-4
  3. If only one resident is operating with an attending, FA should only be chosen if the attending does the entire case with the resident doing absolutely nothing except observing or holding retractors. If the resident participates in the operation other than by observing or holding retractors, SJ (yrs 1-4) or SC (yr 5) should be chosen
  4. In this residency, FA should be used very infrequently unless 2 residents are operating on the same case. If residents are using FA frequently on a given service, the Program Director should be informed about this
  5. 5th yr residents should almost never use FA. The Program Director should be informed of all instances in which a 5th yr resident uses FA

If two residents participate in the same case (one resident is a 5th year, the other is more junior), one of two scenarios will happen:
a. The 5th yr resident will choose SC, and the more junior resident will choose FA
b. The 5th yr resident will choose TA, and the more junior resident will choose SJ
c. The choice of scenario a or b above is up to the 5th yr resident

If two residents participate in the same case (neither resident is a 5th yr), one of three scenarios will happen:
a. The more senior resident will choose SJ, and the more junior resident will choose FA
b. The more senior resident will choose TA, and the more junior resident will choose SJ
c. If both residents are in the same year, one will choose SJ, and the other will choose FA
d. The choice of scenario a or b above is up to the more senior resident
